Co-worker evaluation forms

I've been tasked with coming up with an internal employee feedback form, and have taken it upon myself to overcomplicate it

If you're an employer trying to put together a formal process where employees are providing feedback on other employees, there are going to be all kinds of incentives in play that don't directly serve your goals. People are going to trend towards the paradigm of customer satisfaction surveys, where anything less than a perfect score is a demerit. People are going to want to be kind. If asked to provide some kind of score, people will differ in how they apply the scale. How can we structure this process in a way that acknowledges the psychology and imprecision in play?

My solution is to also score the reviewer in real time, and force them to adjust their feedback until it falls within standardized parameters.

The form provides the employee being reviewed a score of "0" in every category. It provides the reviewer a number of points equal to the number of categories plus 1. The reviewer must assign these points to categories to modify the reviewed's score positively or negatively, up to two per category. The form is accepted when all points have been allocated, and the sum of all score modifications is between -1 and 1.
